Job summary

Trilogy Health Services is seeking a MDS Coordinator (LPN/RN) at Wellbrooke of Wabash in Indiana. You will oversee resident assessments and care planning, ensuring compliance with federal and state regulations for Medicare/Medicaid reimbursement.

Key duties include conducting MDS assessments, evaluating medical data, preparing timely reports, and developing written care plans for each resident. CPR certification and an active LPN/RN license are required.

Qualifications

• Experience with MDS assessments and care planning.
• Current CPR certification required.
• Licensed practical nurse (LPN) or registered nurse (RN) in good standing.

Responsibilities

• Conduct MDS assessments to evaluate resident status and triggers.
• Evaluate condition and data to determine need for additional assessments.
• Prepare and transmit reports to Medicare/Medicaid databases.
• Develop preliminary and comprehensive care plans for residents.
• Inform residents/families about Medicare/Medicaid and financial assistance.
• Ensure MDS notes accurately reflect services and resident response.
• Assist with care plan portion of discharge planning.
