Job summary

Monarch Healthcare Management is seeking an LPN in Minnesota to support MDS coordination, ensure timely completion, and uphold regulatory compliance. You will edit, lock and transmit MDS assessments and work with the Interdisciplinary Team to develop care plans.

Qualifications

• Must be a Licensed Practical Nurse (LPN) in Minnesota.
• CPR certification or willingness to obtain.
• Ability to communicate verbally and in writing when directing the RAI process and when performing care for residents.
• Must perform essential functions of the position with or without reasonable accommodation.
• Must qualify for employment after criminal background check per Minnesota DHS.

Responsibilities

• Edit, lock and transmit MDS assessments to ensure timely completion and accuracy.
• Oversee MDS submission, PDPM and case-mix considerations for reimbursement.
• Coordinate care plans with the Interdisciplinary Team (IDT) to ensure comprehensive resident care.
• Assist staff with MDS coding and clinical documentation improvements.
• Promote regulatory compliance and maintain high quality measures through IDT collaboration.
