Job summary

Selectis Health, Inc. is seeking an MDS/Care Plan Coordinator to oversee resident assessments and ensure compliant coding for CMS billing. You will coordinate MDS development, RAPs, and care plan documentation with the interdisciplinary team in a long-term care facility in Oklahoma City.

The role emphasizes accuracy, timely submissions, and collaboration with physicians, nursing staff, and auxiliary teams to optimize reimbursement while delivering quality resident care.

Qualifications

• Registered Nurse or Licensed Practical/Vocational Nurse with required state licensure.
• Minimum three (3) years of clinical experience in a health care setting.
• Minimum of two (2) years experience in a long-term care setting.
• Familiar with MDS 3.0 and have an in-depth working knowledge of the RAI user manual and RUG categories.
• Excellent analytical and deductive reasoning skills.
• Organized and detailed in work performance.
• Computer literacy and comprehensive understanding of documentation software system.
• Excellent communication skills and genuine caring for and interest in the elderly and disabled in a nursing facility.

Responsibilities

• Oversee and coordinate the development and completion of the resident assessment (MDS) in accordance with current Federal and State rules, regulations, and guidelines.
• Coordinate MDS submission and ensure timeliness and accuracy of documentation.
• Assist in scheduling and conducting Interdisciplinary Care Plan meetings and ensure signatures and approvals are obtained.
• Maintain confidentiality of resident records and ensure documentation integrity across the care team.
