Job summary

SanStone Health & Rehabilitation seeks an RN or LPN MDS Coordinator to lead MDS assessments and care plans for residents using RAI processes in compliance with regulations. This role emphasizes PDPM and accurate data management across the care team.

The position requires strong English communication, attention to detail, and the ability to collaborate with multiple departments. On-site work in NC with opportunities for professional growth and a supportive team environment.

Qualifications

• Current RN or LPN license issued by the State Board of Nursing.
• Experience with the RAI process.
• Strong knowledge of PDPM and State Case Mix Time weighted Reimbursement.
• Familiarity with long-term care laws, regulations, and guidelines.
• Effective English communication and customer service orientation.
• Detail-oriented with strong organizational and multitasking abilities.
• Ability to collaborate with staff and vendors.

Responsibilities

• Coordinate the completion of the MDS by all disciplines.
• Maintain and follow a schedule of due dates for all MDSs.
• Review MDS data for accuracy and meet with staff as needed.
• Complete MDS and input into the computer.
• Ensure MDS transmissions are approved by the State in a timely manner.
• Correct rejected records and prepare them for re-submission.
• Formulate Resident Care Plans with the interdisciplinary team.
