The MDS Coordinator at Bethlehem Woods (American Senior Communities) oversees the Resident Assessment Instrument (RAI) process and interdisciplinary care planning to ensure regulatory compliance. Qualifications

• Graduate of an accredited school of nursing (BSN preferred).
• Minimum of one year in nursing management in the long-term industry.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric settings.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI) and interdisciplinary care planning.
• Ensure compliance with state and federal regulatory requirements.
• Collaborate with nursing, therapy, and support staff to develop and implement resident-centered care plans.
