Job summary

Bethlehem Woods Nursing & Rehabilitation in Fort Wayne, IN is seeking an experienced RN for the MDS Coordinator role. The successful candidate will coordinate the RAI process and drive interdisciplinary care planning to ensure regulatory compliance and high-quality resident care.

Requirements include a BSN preferred, at least one year in nursing management in long-term care, and a minimum of two years nursing experience in related settings. C.A.R.E. values are essential.

Qualifications

• Graduate of an accredited school of nursing, preferably BSN.
• Minimum of one year in nursing management in the long-term industry.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric nursing setting.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI).
• Oversee interdisciplinary care planning to ensure quality resident care.
