Responsibilities

• Complete and coordinate the RAI/MDS assessment process for assigned residents, ensuring accuracy and timely submission.
• Develop comprehensive resident care plans at admission and with changes to improve outcomes.
• Coordinate with the IDT and lead MDS-related meetings and documentation.
• Monitor and audit clinical documentation for regulatory compliance and proper coding.
• Support reimbursement by ensuring correct MDS coding, diagnoses, and PDPM data.
• Maintain knowledge of MDS/RAI requirements and prepare updates for insurers.
