Transitions Healthcare Shook Home is seeking a Nurse Assessment Coordinator (RNAC) to manage the resident assessment process and provide case management within regulatory guidelines. You will lead interdisciplinary teams, coordinate MDS/CAA completion, and ensure timely transmission for compliant billing.

An emphasis on ongoing nursing education, on-call coverage, and participation in care planning will support patient well-being and regulatory adherence.

Qualificações

• RN in good standing with the Commonwealth of Pennsylvania.
• Two years of experience in institutional resident care preferred.
• Knowledge of MDS 3.0.
• CPR and AED certification.

Responsabilidades

• Coordinate development and completion of resident assessments per Federal/State regulations and policy.
• Provide case management.
• Lead or participate in PPS meetings to ensure federal billing compliance.
• Oversee MDS, CAA, and Care Plan completion per regulations.
• Communicate MDS schedules to Interdisciplinary Team and payer sources.
• Verify MDS sections are complete and accurate.
• Ensure timely MDS transmission and ARD validation.
• Develop interdisciplinary plan of care from MDS/CAA data.
