Job summary

Care Pavilion Nursing and Rehabilitation is now hiring a full-time Registered Nurse Assessment Coordinator (RNAC). Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) are both eligible for this role.

The RNAC oversees OBRA and Medicare MDS assessments, ensuring timely completion, submission, and corrections to meet federal regulations. Coordinates interdisciplinary participation and maintains documentation to support compliant care.

Qualifications

• Graduate of accredited nursing program.
• Active PA RN or LPN license.
• Minimum one year of clinical experience; MDS experience preferred.

Responsibilities

• Integrates nursing, dietitian, and interdisciplinary services to ensure accurate assessments.
• Maintains an accurate schedule of MDS assessments with correct reference dates.
• Oversees electronic submission of all MDS assessments and performs corrections as needed.
