Job summary

Caring Heart Rehabilitation and Nursing Center in Philadelphia, PA, is seeking a Registered Nurse Assessment Coordinator (RNAC) to coordinate resident assessments and care planning. You will lead MDS processes, collaborate with the care team, and ensure compliant, patient-centered care.

The ideal candidate holds PA RN licensure, excels in communication and critical thinking, and thrives in a fast-paced rehab/long-term care setting.

Qualifications

• Current RN licensure in Pennsylvania (PA).
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
• Strong problem-solving and organizational skills.
• Experience in rehabilitation or long-term care is a plus.

Responsibilities

• Conduct comprehensive assessments of residents' physical, emotional, and psychological needs.
• Coordinate and complete MDS care plans per state and federal regulations.
• Participate in QAPI and IDT meetings.
• Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making.
• Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
• Participate in quality improvement initiatives to drive continuous improvement and excellence.
• Collaborate with the care team to address resident concerns and needs in a timely and respectful manner.
