Job summary

OPCO Skilled Management is seeking an experienced MDS Coordinator to join a team handling the RAI and care management processes from admission to discharge. The role requires relocation to New Mexico and offers a sign-on bonus and relocation package.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and the MDS process.
• Experience with MDS 3.0 preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Decide on completing or not completing additional MDS assessments per criteria.
• Assist with daily stand-up meeting and set assessment reference dates.
• Comply with federal and state regulations for the RAI process.
• Monitor MDS and care plan documentation in the medical record.
• Maintain current MDS status per state and federal guidelines.
• Ensure timely data entry into MDS programs.
• Code MDS accurately from chart reviews and interviews.
• Attend interdisciplinary meetings and quality assurance sessions.
• Assist DON with significant changes and physician orders to reflect updates.
