Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to ensure timely, accurate RAI and care management from admission to discharge in accordance with company policy and Federal, State and Certification guidelines. The role requires RN licensure and 2+ years in a Skilled Nursing Facility, with relocation to New Mexico to support multiple facilities in Alamogordo, Roswell, Gallup, Farmington, Taos, Las Cruces and Grants.

Qualifications

• RN licensed to practice in the state of the job.
• Minimum 2 years nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, and MDS processes.

Responsibilities

• Assess resident needs with Interdisciplinary Team; schedule MDS assessments and care plan reviews.
• Decide on necessity of additional MDS assessments per RAI User’s Manual.
• Coordinate daily stand-up meetings; review resident care and assessment references dates.
