Job summary

Green Meadows Nursing Center seeks an experienced MDS Coordinator to manage the RAI process and care management from admission to discharge in compliance with Federal, State, and Certification guidelines. Under the supervision of the Director of Nursing and VP of Clinical Reimbursement, you may also coordinate information systems operations and staff education within the clinical department.

The position requires RN licensure in the state of practice, knowledge of MDS 3.0, and strong familiarity

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.

• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• 0
• Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement requirements.
• 0
• Experience with MDS 3.0 is preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the interdisciplinary team.
• 0
• Decide on completing or not completing additional MDS assessments based on clinical criteria.
• 0
• Assist with daily stand-up meetings and set assessment reference dates.
• 0
• Ensure compliance with federal/state regulations for MDS completion and coordination.
• 0
• Maintain MDS status and accurate data entry in computerized systems.
• 0
• Attend team meetings to communicate changes and update records.
• 0
• Support identification of significant changes and communicate physician orders for MDS updates.
• 0
• Prepare scheduling, notices for care planning conferences, and inform staff and families as needed.
• 0
• Maintain knowledge of data entry, computer technology, and ensure timely state submissions.
• 0
• Perform other duties as assigned.
