Job summary

Regency Integrated Health Services seeks a skilled Care Management professional to coordinate the RAI process and ensure accurate MDS documentation for residents in a nursing facility.

You will collaborate with the interdisciplinary team to optimize care plans, manage ARD references, and support Medicare/Medicaid reimbursement requirements while maintaining compliance and data integrity.

Qualifications

• Graduate of an accredited school of nursing with current RN or LVN licensure.
• Experience with MDS/RUGs processes and reimbursement is desirable.
• Ability to lead interdisciplinary teams and coordinate RAI activities.

Responsibilities

• Coordinate the RAI process to ensure timely, accurate resident assessments.
• Ensure MDS/Case Mix documents support reimbursement and compliance.
• Collaborate with the interdisciplinary team to develop and manage care plans.
• Maintain schedule and ARD references for ongoing assessments.
• Perform regular audits of MDS submissions and documentation.
