Job summary

Batavia Rehab and Healthcare Center is seeking an MDS Coordinator | RN to manage the RAI/MDS process from admission to discharge in accordance with company policy and applicable Federal/State guidelines. Collaboration with the Director of Nursing and VP of Clinical Reimbursement will support coordination of information systems and clinical education.

Qualified candidates hold an active RN license in Illinois, have MDS experience, and a background in LTC/SNF/AL/MC settings.

Qualifications

• Active RN license in the state of practice.
• Prior MDS experience preferred in long-term care settings.
• Strong understanding of MDS, RAI, and care planning.
• Knowledge of OBRA, Medicare PPS, and Medicaid concepts.
• Experience with care coordination and regulatory guidelines.
• Excellent written and verbal communication skills.

Responsibilities

• Oversee timely and accurate completion of the RAI/MDS process from admission to discharge.
• Coordinate information systems and education for the clinical team under Nursing leadership.
• Ensure compliance with company policies and Federal/State guidelines.
