Job summary

SNF Staffing Solutions seeks an MDS Coordinator to manage RAI and care plans from admission to discharge, aligned with federal, state and certification guidelines.

Under direction from Nursing leadership, schedule MDS assessments, track dates, and ensure accurate coding and documentation in resident records.

You will collaborate with the Interdisciplinary team and ensure compliance while maintaining confidentiality and data integrity.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.

Responsibilities

• Works with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decides on completing additional MDS assessments based on clinical criteria per the RAI User’s Manual.
• Coordinate daily stand-up meetings and set assessment reference dates.
• Comply with federal and state regulations regarding MDS processes.
• Monitor MDS and care plan documentation in the medical record to support MDS coding.
