Job summary

Outfield Healthcare Partners is seeking an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets Federal, State, and Certification guidelines. You will coordinate the RAI process and oversee the timeliness and completeness of MDS, CAAs, and the interdisciplinary care plan.

The role requires RN or LVN licensure in the state, and typically 2+ years of skilled nursing experience with strong knowledge of Medicare/Medicaid reimbursement and

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Familiarity with OBRA regulations and MDS processes.

Responsibilities

• Actively participates in regulatory or certification survey and correction of deficiencies.
• Reports trends from audits to the Quality Assurance Committee.
• Ensures completion of the RAI Process from MDS to the interdisciplinary plan of care.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms via PointClickCare.
• Follows up with staff to ensure compliance with documentation standards.
• Completes patient assessments, data collection, and staff interviews as needed.
• Facilitates accurate Assessment Reference Date determination reflecting patient care needs.
• Ensures timely submission of MDSs to the State with validation follow-up and maintains records.
