Qualifications

  • 2+ years of nursing experience, preferably in geriatrics.
  • Must maintain an active RN license throughout employment.

Responsibilities

  • Manage completion and submission of MDS Assessments and Care Plans.
  • Educate staff on CMS RAI Manual updates and regulations.
  • Coordinate interdisciplinary MDS assessments and report patient condition changes.

Job description

$10,000 Sign on bonus available for Full Time RN MDS nurse! Seeking an experienced MDS Nurse who wants to work in a team atmosphere that is strong in heart, generous in spirit, and eager to make a difference in our residents' lives.

We are conveniently located off US 131, north of Kalamazoo or a short commute south of Grand Rapids.

Position Summary:

The RN MDS Coordinator manages the completion and submission of accurate and timely interdisciplinary MDS Assessments, CAAs, and Care Plans, adhering to CMS RAI Manual Regulations and all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements:
  • Associate’s or bachelor’s degree in nursing from an accredited institution
  • Active RN license in applicable state, maintained throughout employment
  • Two (2) years’ nursing experience; geriatric experience preferred
  • CRN C Certification
  • CPR certification upon hire or during orientation, maintained current during employment
Specific Job Requirements:
  • Advanced knowledge in the field of practice
  • Ability to make independent decisions as needed
  • Knowledge of practices, procedures, laws, regulations, and guidelines in post-acute care
  • Implement and interpret departmental programs, policies, and procedures
  • Proficiency in all competency areas including patient rights, safety, and sanitation
  • Maintain professional relationships with all staff and vendors
  • Maintain confidentiality of proprietary and confidential information
  • Follow company policies, including harassment and compliance procedures
  • Adhere to Life Care’s Code of Conduct and complete mandatory training
Essential Functions:
  • Coordinate and assist with interdisciplinary MDS assessments, CCAs, and Care Plans
  • Report patient condition changes to the DON
  • Educate staff on updates to CMS RAI Manual or regulations affecting documentation
  • Review and assist with the Interdisciplinary Care Plan
  • Review CMS Reports and ensure assessments are completed and accepted into CMS QIES system
  • Attend relevant meetings and perform staff nurse functions as required
  • Provide excellent customer service and maintain a positive attitude
  • Assist with patient evacuation when necessary
  • Maintain dependable attendance and demonstrate good judgment
  • Communicate effectively within an interdisciplinary team
  • Perform physical activities as needed during work hours
  • Proficient in English language skills
