RN Field Clinical Care Coordinator, Putnam and Jackson Counties, TN

Join us at UnitedHealthcare as an RN Field Clinical Care Coordinator for Putnam and Jackson Counties, TN. This role involves managing care for members with complex medical and behavioral needs, focusing on supporting their medical, behavioral, and socioeconomic requirements to improve health outcomes.

This is a home-based position with field responsibilities in Cookeville, Gainsboro, and surrounding areas.

Primary Responsibilities

1. Engage members face-to-face and/or via phone to conduct comprehensive needs assessments covering medical, behavioral, functional, cultural, and socioeconomic aspects.
2. Develop and implement person-centered care plans addressing chronic conditions, health promotion, social determinants, medication management, and safety, aligned with evidence-based guidelines.
3. Collaborate with internal care teams, providers, and community resources to execute care plans.
4. Provide education and coaching to support member self-management and lifestyle changes.
5. Support discharge planning and coordinate care transitions following ER visits, hospital stays, or SNF admissions.
6. Advocate for members and their families to ensure their needs and preferences are fully supported.

Qualifications

Required

• Undergraduate degree or equivalent experience
• Unrestricted RN license in Tennessee
• At least 2 years of clinical experience
• Experience with MS Office (Word, Excel, Outlook)
• Reliable transportation with ability to travel up to 75%

Preferred

• BSN, Master’s or higher in clinical field
• CCM certification
• Community case management experience with complex needs
• Experience in team-based care
• Background in managed care

The hourly pay ranges from $28.61 to $56.06, based on factors like experience, education, and location. Benefits include comprehensive packages, incentives, stock purchase options, and 401k contributions.
