Remote Patient Monitoring and Chronic Care Management Coordinator (LPN or RN)

Hannibal, MO

Department: Care Management

Schedule: Full-Time, Days, Mon-Fri, 8am-4:30pm

Job Responsibilities

Responsible for patient enrollment services related to digital remote patient monitoring (RPM) programs for chronic diseases such as, but not limited to, Heart Failure, COPD, and Diabetes. Duties include digital platform enrollment, alert monitoring, patient outreach and education, care coordination and escalation, and communication with providers and the multidisciplinary team. Also responsible for patient enrollment services for Chronic Care Management, including outreach and education.

Minimum Qualifications
  • Graduate of an accredited School of Nursing.
  • Current LPN or RN license in Missouri.
  • Excellent verbal and written communication skills.
  • Strong assessment, time management, and critical thinking skills.
  • Ability to interact effectively with diverse individuals and teams at all organizational levels.
