Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six months or less, serving individuals in their home, nursing facilities, and in the hospital. About Hospice

In collaboration with the interdisciplinary team, assess, plan, implement, coordinate, monitor, and evaluate strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through the nursing process, relationship-centered care principles, and a holistic philosophy, ensuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, and sacred death.

Scope of work

Under the direct supervision of the Nursing Manager and in collaboration with the interdisciplinary team, the Nurse uses clinical judgment skills to assess and implement the treatment plan for patients. The RN assesses, monitors, detects, plans, and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatments.

Qualifications

Required

• Associate's degree in nursing
• 2 years of relevant experience
• Michigan Registered Nurse (RN) license upon hire
• State of MI Driver's License upon hire
• BLS - AHA or ARC within 90 days

Preferred

• Bachelor's degree in nursing
• 1 year of relevant experience in acute care, emergency, or rehab facility experience for all of the Home Care business lines
