Job summary

PACS Group seeks a Nurse Manager to oversee facility operations ensuring PDPM and MDS processes align with guidelines. You will coordinate interdisciplinary teams, monitor PMDS timelines, and guide staff through compliant care planning. Strong leadership and regulatory knowledge are essential.

The role emphasizes accurate data entry, audit readiness, and continuous improvement of resident care outcomes in a skilled nursing setting. Requires BSN and state licensure.

Qualifications

• BSN preferred; must be in good standing with State Board of Nursing and maintain continuing education/licensing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficiency in Microsoft Office suite and PCC software is required.

Responsibilities

• Oversee facility weekly PDPM meetings for skilled patients.
• Monitor utilization of PDPM tools and QM/QASP performance with data analysis and recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change.
• Coordinate interdisciplinary participation for MDS and care plans within regulatory timeframes.
• Maintain schedule of all MDS assessments with proper reference dates.
• Handle data entry to ensure accurate MDS submissions and records.
• Verify electronic MDS submissions and perform necessary corrections.
