Qualifications

• Bachelor of Science in Nursing, active licensure in good standing.
• Minimum 3 years of supervisory experience in a long-term care setting.

Responsibilities

• Oversee facility PDPM/ MDS processes and weekly PDPM meetings.
• Ensure accurate and timely completion of all MDS assessments and related documentation.
• Coordinate interdisciplinary participation in MDS and care planning with regulatory compliance.
