Focused Post Acute Care Partners FPACP is seeking an experienced MDS Coordinator to assist the Director of Nursing. You will ensure documentation complies with federal, state, and certification guidelines and coordinate the RAI process for MDS, CAAS, and care plans.

The role requires LVN licensure and solid nursing experience in skilled nursing. The team emphasizes ROCKStar recognition and a supportive work culture, with DailyPay access and full benefits.

Qualifications

• Must have a current LVN license.
• Minimum of two (2) years nursing experience in skilled nursing facility preferred.
• Excellent knowledge of RAI Process, the federal Medicare PPS process, and Medicaid reimbursement as required.
• Thorough understanding of the Quality Indicator process.
• Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care plan process.

Responsibilities

• Assure resident safety and regulatory adherence.
• Coordinate RAI process from MDS through care plan completion.
• Monitor and report trends from audits to Quality Assurance.
• Ensure timely submission of MDS and care plans to state and federal agencies.
• Assign and instruct staff in the RAI process, PPS, and clinical software.
• Maintain confidentiality and accurate documentation.
