The role requires a California LVN license and at least 1 year of MDS experience in SNF or post-acute settings, with strong documentation and organizational skills.

Qualificações

• Must hold a current California LVN license.
• Minimum 1 year of MDS experience in a SNF or post-acute setting.
• Knowledge of MDS 3.0, RAI process, PDPM, Medicare and Medicaid reimbursement.

Responsabilidades

• Coordinate and manage the MDS/RAI process per CMS and state/federal regulations.
• Complete accurate and timely MDS assessments and documentation.
• Develop and update resident care plans.
• Collaborate with nursing, therapy, dietary, and interdisciplinary teams.
• Monitor PDPM documentation and reimbursement accuracy.
• Track quality measures and identify opportunities for clinical improvement.
• Assist with survey preparation, audits, and compliance initiatives.
• Educate and support staff on MDS processes and documentation requirements.
• Partner with facility leadership to ensure clinical and operational excellence.
