Outfield Healthcare Partners seeks an experienced MDS Coordinator to join our fully-staffed facility. The successful candidate will oversee the RAI process from admission to discharge, while coordinating with the Director of Nursing and VP of Clinical Reimbursement.

You will ensure regulatory compliance, accurate MDS coding, and timely care plan updates. RN or LVN license in the state and 2+ years SNF experience are preferred. Equal opportunity employer.

Qualificações

• Graduate of RN or LVN program and licensed in the state of practice.
• Minimum 2 years nursing experience in a Skilled Nursing Facility.
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding OBRA regulations and MDS processes.

Responsabilidades

• Coordinate timely and accurate completion of the RAI process from admission to discharge.
• Collaborate with Interdisciplinary Team for MDS assessments and care plan reviews.
• Ensure documentation supports MDS coding in medical records.
• Monitor MDS status and data entry in appropriate programs.
