Job summary

Southwest-Lt in Oklahoma City seeks a qualified MDS Coordinator to manage the nursing aspects of the MDS process, Medicare compliance, admissions and care plans. You will coordinate MDS schedules, ensure accurate documentation and collaborate with the interdisciplinary team to optimize care and regulatory compliance.

The role requires an active RN or LVN license, long-term care experience and strong communication with residents, families and staff.

Qualifications

• Must be a Registered Nurse or Licensed Vocational Nurse with valid licensure in the state of employment.
• Experience in long-term care and Medicare nursing documentation is preferred.
• Ability to work with interdisciplinary teams and coordinate care plans.

Responsibilities

• Coordinate MDS schedules and pre-admission screening.
• Ensure timely, accurate Medicare nursing documentation and MDS coding.
• Collaborate with interdisciplinary team, residents and families on care plans.
• Monitor RUG assignments and ensure compliance with regulatory requirements.
• Assist with admissions, discharges and look-back period processes.
