Job summary

Reliant Management is seeking an experienced MDS Coordinator to direct and organize resident assessments for Medicare/Medicaid reimbursement. You will collaborate with interdisciplinary teams to ensure timely MDS completion and accuracy in documentation.

The ideal candidate holds a California LVN/RN license, has at least 1 year of MDS experience, and is proficient in English with CPR certification preferred. Knowledge of PointClickCare is a plus.

Qualifications

• Licensed LVN or RN in California.
• Minimum 1 year experience as an MDS Nurse.
• Written and verbal English communication for business.
• CPR certification preferred.
• Knowledge of PointClickCare.

Responsibilities

• Coordinate and direct the MDS process with the interdisciplinary team.
• Calculate Medicare assessments and PPS dates.
• Ensure documentation complies with Medicare and California reimbursement rules.
• Review medical records to gather MDS data and status.
• Attend care plan conferences and related meetings.
• Monitor data accuracy, timeliness, and compliance with standards.
• Stay current with regulations and continuing education.
