Job summary

OPCO Skilled Management seeks an MDS Coordinator to support the Director of Nursing and the RN Assessment Coordinator. This role ensures documentation aligns with Federal, State, and Certification guidelines and coordinates the RAI process for timely MDS, CAAs, and the Interdisciplinary Care Plan.

The position requires RN or LVN licensure in the state and at least 2 years of nursing experience in a skilled nursing setting, with strong knowledge of Medicare PPS, Medicaid reimbursement, OBRA, and

Qualifications

• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Understanding OBRA regulations and Minimum Data Set (MDS) processes.
• Familiarity with care planning processes and quality indicators.

Responsibilities

• Assists the Director of Nursing and RN Assessment Coordinator to ensure documentation meets Federal, State, and Certification guidelines.
• Coordinates the RAI process to assure timeliness and completeness of MDS, CAAs, and the Interdisciplinary Care Plan.
• Monitors MDS submissions, validates data, and maintains tracking records; supports QA activities and survey readiness.
