Job summary

Brickyard Healthcare is seeking an MDS Coordinator (LPN/RN) to lead the resident assessment process in compliance with federal and state regulations and company policies. The role acts as an in-house case manager, coordinating with physicians, families, and staff to ensure timely and accurate MDS completion and care planning.

The ideal candidate holds an active LPN or RN license, is CPR certified, and has at least two years of clinical experience in long-term care.

Qualifications

• Responsible for completion of the Resident Assessment Instrument per federal and state regulations and company policy
• Acts as in-house case manager coordinating services with physicians, families, payers and staff
• Coordinates development and completion of the MDS in accordance with rules and guidelines

Responsibilities

• Completes the Resident Assessment Instrument per regulations
• Coordinates MDS processes and resident care planning
• Communicates with care teams to ensure accurate billing and care delivery
• Ensures correct ARD and RAPs per Medicare/Medicaid rules
• Maintains regular attendance and CPR certification
