Job summary

Complete Care at Brick is seeking a Registered Nurse-RN-MDS Coordinator to join our team and lead accurate MDS assessments and individualized care planning in a skilled nursing setting. You will work with an interdisciplinary team to ensure regulatory compliance and high-quality resident outcomes.

The role requires an active New Jersey RN license and a background in long-term care, with strong clinical and communication skills to educate staff on MDS processes and documentation standards.

Qualifications

• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey RN license.
• Long-term care experience preferred.
• RN-MDS coordinator or MDS experience in skilled nursing preferred.
• Strong clinical, communication and organizational skills.
• Knowledge of federal and state MDS regulations.

Responsibilities

• Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines.
• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as residents' conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
• Work closely with leadership to maintain compliance and promote continuous quality improvement.
• Help ensure Complete Care at Brick continues to provide the highest level of resident-centered care.
