Job summary

Dart Healthcare Staffing is seeking an MDS Coordinator to lead the MDS process and utilization management in a long-term care setting. The role focuses on ensuring accurate documentation, regulatory compliance, and effective care planning for Guests.

Responsibilities include coordinating care plans, auditing documentation, assisting with discharges, and maintaining confidentiality. The position requires RN or LPN licensure and experience in a long-term care environment, with a 24/7 facility

Qualifications

• Current State License as a Registered Nurse or Licensed Practical Nurse.
• One year experience as an RN or LPN in a long term care setting preferred.
• Knowledge and training in all aspects of MDS process.

Responsibilities

• Keep abreast of current federal and state regulations and professional standards.
• Assist with development of comprehensive care plans coordinated with the MDS.
• Coordinate, manage and monitor the written plan of care for each Guest.
• Assist nursing management with the coordination, management, and review of nurse's notes to ensure care plan compliance.
• Monitor Guest status changes to ensure appropriate nursing involvement.
• Perform regular audits of documentation to assure accuracy and regulatory compliance.
• Assist nursing management with the discharge process and complete required forms and reports.
• Maintain infection control practices and confidentiality of Guest information.
