Job summary
LionStone Care in Middletown, OH is seeking an MDS Coordinator - LPN to coordinate and oversee the MDS assessment process in accordance with federal and state regulations. Collaborate with the interdisciplinary team to develop and implement individualized care plans based on MDS assessments and ensure timely CMS submissions.
Requires an active LPN license and 2–3 years of clinical nursing experience, with 1 year in long-term care preferred and MDS knowledge.
Qualifications
- Education: Graduate of an accredited LPN program.
- Licensure: Active and unrestricted LPN license.
- Experience: 2-3 years of clinical nursing, 1 year in long-term care; MDS experience preferred.
Responsibilities
- Coordinate and submit accurate MDS assessments per regulatory timelines.
- Develop and implement individualized care plans with the interdisciplinary team.
- Ensure regulatory compliance and timely CMS submissions for reimbursement.
- Serve as liaison with residents and families regarding MDS and care plans.
- Collaborate with nursing and therapy staff to gather MDS data and monitor progress.
- Support quality improvement initiatives using MDS data.
