Job summary

Blue Ridge Health and Rehabilitation is seeking an experienced MDS Coordinator (RN/LPN) to lead the Resident Assessment Instrument process in compliance with Federal and State regulations and company policy. The role requires coordinating MDS documentation, leading Interdisciplinary Team reviews, and ensuring timely transmission for accurate billing, with a focus on quality, compliance, and patient care.

Qualifications

• Must possess current RN or LPN license and CPR certification.
• Minimum three years of clinical experience; long-term care preferred.
• Strong verbal and written English communication and organizational skills.

Responsibilities

• Meet physical and sensory requirements and work in the described environment.
• Lead or participate in Daily PPS, weekly Medicare, and month-end meetings to ensure billing requirements.
• Oversee MDS coordination (MDS, CAAs, Care Plans) per Federal/State regulations.
• Coordinate MDS data collection via medical records, bedside assessments, interviews.
• Ensure ISDT has timely, complete, and signed assessments.
• Review MDS prior to closing and transmission for compliance.
• Develop Interdisciplinary Plan of Care from MDS and CAAs; review quarterly.
