Complete Care at Brick is hiring a Registered Nurse-RN-MDS Coordinator in New Jersey to ensure accurate MDS assessments and individualized care plans for residents. You will collaborate with an interdisciplinary team to drive quality outcomes and regulatory compliance.

Qualifications

• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey RN license.
• Long-term care experience preferred.
• Experience as RN-MDS Coordinator or MDS assessment in a skilled nursing setting preferred.
• Strong clinical assessment, communication, and organizational skills.
• Knowledge of federal and state MDS regulations.
• Ability to work collaboratively with interdisciplinary teams.

Responsibilities

• Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines.
• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as residents' conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
• Work closely with leadership to maintain compliance and promote continuous quality improvement.
• Help ensure Complete Care at Brick continues to provide the highest level of resident-centered care.
