Hawthorn Glen Care Community in Middletown, OH is seeking an experienced MDS Coordinator (LPN) for a full-time role. Manage MDS assessments, care planning, and regulatory compliance to support resident care and reimbursement.

Collaborate with an interdisciplinary team and communicate with residents and families. The ideal candidate holds an LPN license with 2–3 years of clinical experience, including long‑term care, and has strong organizational and documentation skills.

Qualifications

• Graduate of an accredited LPN program.
• Active and unrestricted LPN license.
• 2–3 years of clinical nursing experience; 1 year in long‑term care preferred; MDS experience preferred.

Responsibilities

• Coordinate accurate and timely MDS assessments per regulatory timelines.
• Develop and update individualized care plans with interdisciplinary team.
• Ensure compliance with federal/state MDS/RAI regulations and CMS submissions.
• Serve as liaison to residents and families on MDS and care planning.
• Collaborate with nursing, therapy, dietary, and social services.
• Monitor data for quality improvement and address audit findings.
