Arlington-Pointe-Care-Center in Middletown, OH is seeking an MDS Coordinator/LPN to oversee the MDS assessment process in compliance with federal and state regulations. You will coordinate timely MDS submissions, collaborate with the interdisciplinary team, and ensure accurate care plans and revenue alignment for residents.

The role requires an active LPN license, 2–3 years of clinical nursing experience (1 year in long-term care preferred), strong assessment and documentation skills, and

Qualifications

• Graduate of an accredited LPN program.
• Active and unrestricted LPN license.
• 2–3 years clinical nursing experience, 1 year in long-term care preferred.
• Experience with MDS assessments preferred.
• Strong documentation and communication skills.

Responsibilities

• Coordinate completion and submission of MDS assessments for all residents.
• Review MDS data for accuracy and completeness before submission.
• Collaborate with interdisciplinary team to develop individualized care plans.
• Participate in care plan meetings and update plans as needed.
• Ensure compliance with MDS, RAI, and CMS timelines.
• Serve as liaison to residents and families regarding MDS and care planning.
• Monitor MDS data for quality improvement and address deficiencies.
