LionStone Healthcare in the Columbus area (Dublin, OH) is seeking a skilled MDS Coordinator - LPN to oversee MDS assessments and care planning in a long-term care facility. This role emphasizes accurate data, regulatory compliance, and collaborative care planning with an interdisciplinary team.

The ideal candidate has an LPN license with 2–3 years of clinical nursing experience, including exposure to MDS processes, and strong organizational and communication skills.

Qualifications

• Graduate of an accredited LPN program.
• Active and unrestricted LPN license.
• Minimum of 2–3 years of clinical nursing experience, with at least 1 year in long-term care; MDS experience preferred.

Responsibilities

• Coordinate timely and accurate MDS assessments for all residents per regulatory timelines.
• Collaborate with interdisciplinary team to develop and update individualized care plans based on MDS data.
• Maintain knowledge of federal/state MDS/RAI regulations and CMS submission requirements.
• Serve as a primary contact for residents and families regarding MDS and care planning.
• Work with billing/finance to ensure MDS data supports Medicare/Medicaid reimbursement.
• Monitor data quality and participate in quality improvement initiatives.
