Job summary

The Hawthorn Glen Care Community seeks an MDS Coordinator (LPN or RN) to lead the MDS assessment process in compliance with federal and state rules. You will coordinate timely assessments, support care planning, and ensure accurate data for CMS reimbursement within a long-term care setting.

Ideal candidates hold a current LPN or RN license, 2–3 years of clinical nursing experience (1 year in long-term care preferred), and are proficient with PCC EHR and MDS software.

Qualifications

• Graduate of an accredited LPN or RN program.
• Active and unrestricted LPN or RN license.
• 2–3 years of clinical nursing experience, with 1 year in long-term care preferred.
• Experience with MDS assessments is preferred.
• Strong clinical assessment and documentation skills.
• Knowledge of MDS 3.0, RAI process, and related regulations.

Responsibilities

• Coordinate the completion and submission of MDS assessments in accordance with regulatory requirements.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans based on MDS assessments.
• Ensure timely submission of MDS assessments to CMS and monitor for deficiencies.
• Assist in care plan meetings and update resident care plans as needed.
• Communicate with residents and families about the MDS process and care planning.
• Support quality improvement initiatives using MDS data.
