Hawthorn Glen Care Community in Middletown, OH is seeking an MDS Coordinator (LPN or RN) to oversee the MDS assessment process and ensure regulatory compliant care planning in a long-term care setting.

The role requires strong clinical skills, 2–3 years of nursing experience (1 year in long-term care preferred), and proficiency with MDS 3.0, RAI, and PCC EHR/MDS software. Collaboration with the interdisciplinary team is essential.

Qualifications

• Graduate of an accredited LPN or RN program.
• Active and unrestricted LPN or RN license.
• 2-3 years of clinical nursing experience, with 1 year in long-term care preferred.
• Experience with MDS assessments preferred.

Responsibilities

• Coordinate the completion and submission of accurate and timely MDS assessments.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Maintain knowledge of current federal and state regulations regarding MDS, RAI, and Medicare/Medicaid reimbursement.
• Serve as a point of contact for residents and families regarding MDS, care planning, and assessments.
• Work with nursing, therapy, dietary, and social services to address resident needs.
• Monitor and participate in quality improvement initiatives based on MDS data.
