Job summary

LionStone Care in Dublin, OH is seeking an MDS Coordinator - LPN to lead the MDS assessment process for our long-term care residents. You will coordinate timely assessments and collaborate with the care team to optimize care plans and reimbursement where applicable.

Ideal candidates are LPN graduates with 2–3 years of clinical nursing experience, including long-term care, and familiarity with MDS 3.0, RAI, and EHR/MDS software such as PCC.

Qualifications

• Graduate of an accredited LPN program.
• Active and unrestricted LPN license.
• 2–3 years clinical nursing experience, 1 year in long-term care preferred.
• Familiarity with MDS 3.0, RAI, and related regulations (CMS).

Responsibilities

• Coordinate and submit accurate MDS assessments per regulations.
• Collaborate with interdisciplinary team to develop care plans.
• Ensure compliance with federal/state MDS and reimbursement rules.
• Communicate with residents and families about MDS and care plans.
• Participate in team meetings to review progress and outcomes.
• Support quality improvement initiatives using MDS data.
