Job summary

Trilogy Health Services, LLC is seeking an MDS Coordinator (LPN/RN) for our WoodBridge Health Campus in Logansport, IN. You will oversee resident assessments, plan care, and ensure compliance with Medicare/Medicaid requirements, collaborating with the care team to optimize outcomes.

Applicants must hold an active LPN or RN license in Indiana and CPR certification, with 3–5 years in MDS/assessment in long-term care preferred. We offer competitive pay and comprehensive benefits.

Qualifications

• Must hold a current, valid LPN or RN license in Indiana.
• Minimum 3–5 years’ experience in MDS or assessment in long-term care.
• CPR certification required.

Responsibilities

• Conduct and complete the MDS assessment to evaluate residents’ status and implement CAAs.
• Evaluate residents’ condition to determine need for additional assessment activities.
• Prepare and electronically transmit timely reports to Medicare/Medicaid databases.
• Develop a written plan of care (preliminary and comprehensive) for each resident.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
• Ensure MDS notes reflect services provided and residents’ response to care.
• Assist with care plan portions of discharge planning and implement committee recommendations.
