Job summary

Westminster Commons is seeking an MDS Coordinator to manage the Resident Assessment Instrument process. You will ensure accurate MDS, develop the interdisciplinary plan of care, and lead daily PPS/Medicaid case mix meetings. The role requires GA licensure and prior MDS experience.

This hourly, non-exempt position reports to the Administrator and involves coordinating scheduling, assessments, and data transmission while maintaining compliance with federal and state regulations.

Qualifications

• Must possess a current, unencumbered RN or LVN license in Georgia.
• Registered Nurse license preferred.
• 2 years’ long-term care or MDS experience preferred.
• Ability to make independent decisions and lead processes.

Responsibilities

• Coordinate the MDS process to ensure accurate, timely documentation.
• Schedule assessments (OBRA, Medicare PPS, Medicaid) and communicate with departments and families.
• Monitor resident condition and coordinate MDS reassessment and plan of care updates.
• Strategically set ARD with the interdisciplinary team to reflect resident needs.
• Complete MDS and CAAs per policies; attend Casemix and Utilization Review meetings.
• Transmit MDS data electronically to state regulators; maintain records 15 months.
