Sage Event Management Inc is seeking a dedicated MDS Coordinator to ensure accurate assessments, regulatory compliance, and effective care planning for residents. In this role you will oversee admissions and annual MDS assessments, coordinate interdisciplinary teams, maintain data integrity in MDS software, and support quality reporting to leadership.

The ideal candidate holds an active Nursing License, proves geriatric nursing experience, and can manage multiple priorities with reliable

Qualifications

• Current RN license in good standing.
• Experience in geriatric nursing.
• Excellent assessment, documentation, and communication skills.
• Knowledge of nursing home regulations and compliance requirements.

Responsibilities

• Coordinate admission, annual, quarterly, Medicare, and change-of-condition MDS assessments.
• Ensure regulatory compliance with Federal and State guidelines for MDS.
• Review MDS and care plan files for accuracy and organization.
• Maintain data entry and reporting accuracy in MDS software.
• Lead interdisciplinary meetings and coordinate care planning.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data per reporting standards promptly.
• Attend professional development to stay current with industry standards.
• Oversee ancillary data reporting to corporate billing.
