Job summary

Westgate Health & Rehab is seeking a qualified MDS Coordinator to administer patient assessments and manage the assessment process, scheduling, and timely completion. The role ensures compliant MDS, CAAs and care plans in coordination with the care team and payers.

The position requires a Bachelor’s in Nursing, active RN license in Florida, and at least two years of LTC experience, with proficiency in MDS 3.0 and regulatory knowledge.

Qualifications

• Bachelor’s in Nursing is required.
• Proficiency with MDS 3.0 is required.
• Registered Nurse with an active license in the state of employment is required.
• Minimum two years of clinical LTC experience.
• Prior experience as an MDS coordinator is preferred.

Responsibilities

• Oversees accurate completion of MDS, CAAs and Care Plans per current federal and state regulations.
• Acts as in-house Case Manager to develop care pathways and coordinate information flow to residents, families and payers.
• Communicates regulatory risk and system effectiveness to Administrator and Director of Nursing.
• Captures ARD, determines RUG scores and documents care and services provided.
• Ensures timely electronic submission of MDS to the state database and follows up on validation reports.
• Educates IDT members on the RAI process and trains staff on updated MDS processes.
