Job summary

Topaz Fiscal Services LLC seeks an MDS Coordinator to manage the Resident Assessment Instrument process, ensuring timely MDS completion and accurate care planning. You will lead interdisciplinary collaboration and coordinate Medicare PPS/Medicaid case mix activities within a healthcare facility.

The role requires RN or LVN licensure, with strong knowledge of MDS, RAI timing, and regulatory compliance. On-site work in Conyers, GA, with regular meetings and documentation duties.

Qualifications

• Requires current active RN or LVN license in state.
• RN license preferred.
• 2 years’ experience in long-term care preferred.
• 6 months rehabilitation/restorative nursing preferred.
• 2 years’ experience in RAI process completion preferred.

Responsibilities

• Maintain MDS status for each resident in compliance with laws and facility policies.
• Schedule required assessments and coordinate with departments and families.
• Monitor resident condition and coordinate MDS reassessments and care plan revisions.
• Set ARD with interdisciplinary team to reflect resident status accurately.
• Complete MDS and CAAs accurately per policies and guidelines.
• Attend daily casemix and weekly Utilization Review meetings; monitor records for 15 months.
