Job summary

Manchester Center for Rehabilitation and Healing is seeking an MDS Coordinator to join our skilled nursing team. The role focuses on accurate completion of MDS, CAAs and care plans, in line with federal and state regulations, with independent evaluation and timely communication.

Candidates must hold an active nursing license in the state, have at least two years clinical experience (one in long-term care), and have prior MDS coordination experience; training available for qualified assessors.

Qualifications

• Licensed Nurse with current, active license in state of practice.
• Minimum two (2) years of clinical experience in a health care setting.
• Minimum of one (1) year of experience in a long term care setting.
• Prior experience as an MDS coordination accepted.
• Training program available for candidates with demonstrated assessment skills.

Responsibilities

• Oversees accurate and thorough completion of the MDS, CAAs and Care Plans.
• Acts as an in-house Case Manager with communication to resident, family, other professionals and third party payers
• Proactively communicates regulatory risk and effectiveness of facility systems to Administrator and Director of Nursing
• Collaborates with therapy staff to select the correct ARD reason and capture the RUG score
• Demonstrates understanding of MDS requirements for Medicare, Managed Care and Medicaid
• Ensures timely electronic submission of all MDS to the state database and reviews validation reports
• Facilitates the Care Management Process engaging the resident, IDT and family for discharge planning
• Educates IDT members on the RAI process and trains on updated MDS processes
• Analyzes QI/QM data with the Director of Nursing to identify trends
• Responsible for Utilization Review and Triple Check and participates in committees
