Job summary

Life Care Centers of America is seeking an RN/LPN MDS Coordinator to coordinate interdisciplinary MDS assessments, CCAs, and care plans in a Washington facility. You will ensure regulatory compliance and educate care teams on CMS updates.

The role requires an active RN/LPN license in good standing and at least two years of nursing experience, preferably in geriatric settings, with CPR certification either on hire or during orientation.

Qualifications

• Associate or bachelor's degree in nursing.
• Active RN or LPN license in good standing.
• Two years' nursing experience; geriatric preferred.
• CRN C Certification (clinical compliance).
• CPR certification at hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans per CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by the MDS Assessment to the DON.
• Educate direct care staff on CMS RAI Manual updates impacting documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and attest that assessments are ready for CMS QIES submission.
• Attend PPS and Triple Check meetings; perform staff nurse functions as required.
• Maintain patient rights, safety, and confidentiality in all activities.
