Job summary

Life Care Centers of America seeks an RN MDS Coordinator to coordinate and assist with MDS Assessments, CCAs, and Care Plans per CMS RAI guidelines and Life Care standards. You will ensure timely submissions and accuracy across interdisciplinary teams in a post-acute setting.

Qualifications

• RN license in good standing and active.
• 2 years of nursing experience; geriatrics preferred.
• CPR certification required or obtained during orientation.

Responsibilities

• Coordinate and assist with MDS Assessments, CCAs, and Care Plans per CMS RAI guidelines.
• Report changes in a patient’s condition to the DON.
• Educate direct care staff on CMS RAI updates affecting documentation.
• Review Interdisciplinary Comprehensive Care Plan and CMS Reports.
• Attend PPS meetings and other requested meetings.
• Perform staff nurse functions as required.
• Maintain excellent customer service and professional conduct.
• Assist in evacuations and ensure patient safety.
• Maintain regular, dependable attendance; follow policies on confidentiality.
