The Waters Of Tipton in Tipton, IN, is seeking an MDS Coordinator/Nurse (RN or LPN) to perform federally mandated assessments and coordinate comprehensive care plans in a long-term care setting. This role ensures accurate data collection and appropriate plans of care for all payer sources.

You will oversee OBRA, PPS, and MDS processes, coordinate care planning with the IDT, and manage related documentation to support timely patient care and reimbursement.

Formación

• RN or LPN with MDS Coordinator responsibilities
• Knowledge of OBRA, PPS and MDS processes
• Experience coordinating care plans and documentation

Responsabilidades

• Reports to the facility Administrator and completes OBRA, PPS and MDS
• Completes Admit MDS Tracking Forms, discharge assessments, and related documentation
• Coordinates Care Plan conferences and communications with residents and families
• Maintains OBRA calendars, schedules, and MDs/CAAs documentation
• Ensures timely completion of MDS Interviews and ARD period requirements
• Manages CMS transmissions and Medicare/Managed Care documentation
